I've just checked /icons dir on this theme, and it has no os_hsierra.icns, os_moja.icns. So, if you're using HighSierra or Mojave, on Clover GUI it will use generic "os_mac.icns" by default (as seen on your screenshot above). Also if you've installed it using APFS, try following @fusion71au guides here for creating custom entries, so both MainSystem && RecoveryHD show proper icons (this theme already contains os_recovery, vol_recovery icns). It seems, not related to Clover Theme. If what you mean is under 10.13 / 10.14 with APFS, just check it with $ `diskutil list` take a look, does it use "Recovery" only or "Recovery HD" for volumes name? My Mojave beta drive (which is an external SSD attached by USB 3.0), doesn't show the correct drive icon when connected.
